4. 国民健康保険料は納期限までに納めてください
Please pay the National Health Insurance Premium, Kokumin Kenko Hokenryo, by the due date

As National Health Insurance Premium is source of revenue for the insurance
benefit, therefore, it is very important to make payment by the due date without fail. For this payment, account transfer payment is convenient.
■If it is kept unpaid
Except special case such as disaster, if you continue non-payment of the premium, the following measures are conducted.
1) Your health insurance benefit from high-cost medical care and other benefits
2) will be allocated to your unpaid insurance payment.
3) Depending on period of the unpaid premium, the validity term of your health insurance certificate will be shortened, so called “short term health insurance
4) certificate” will be issued instead or “insured qualification certificate” is issued which you have to pay all the medical bills each time temporarily.
3) If you continue the non-payment more than one year and six months, a part
of, or all of, your health insurance benefit may be suspended.
4) Seizure of property may be conducted.
■ Getting or losing the qualification of “National Health Insurance”
1) When you leave your job or newly move into Toyokawa from other places,
please apply for the qualification of “National Health Insurance”, and the insurance premium becomes effective. If application is delayed, you may have to
pay the insurance premium which is calculated back to the day of qualification occurrence, so please complete the registration WITHIN 14 DAYS.
2) The insurance premium will be imposed on householder. A householder is the tax debtor for national health insurance. Even though the householder has the
Social Health Insurance, in case of the family members belonging to National Health Insurance, the notification is addressed to the householder.
5. 児童扶養手当を支払います

Child rearing allowance called JIDOFUYO TEATE in Japanese, subsidizing single
parent families, will be paid to the designated bank account on Friday, September 11. This time, the payment of the allowance is for 2 months (July and
August). The notice is not informed individually. Please check it with your bankbook etc.
6. 障害福祉各種手当の所得状況届
A Report of Annual Income for welfare allowances

Disabled persons who receive following allowance need to submit present condition form of annual income, Shotoku Jokyo Todoke.
Tokubetsu Shogaisha Teate, Shogaiji Fukushi Teate, Keikateki Fukushi Teate, and
Tokubetsu Jido Fuyo Teate. If submission has not been done yet, please submit
the present condition form of annual income by Friday, September 11 to the Social Welfare Division.
7. ロタウイルス定期予防接種
Periodic vaccination against Rotavirus

From Thursday, October 1, vaccination against Rotavirus is added to child periodic
vaccination. This vaccination prevents one of infectious gastrointestinal inflammations with frequent occurrence in infantile period.
Eligibility: Child who was born in August, 2020 and after
Fee: Free of charge
Application: Directly apply to commissioned medical facilities in Toyokawa city.
About commissioned medical facilities, please confirm on City HP.
Others: For eligible persons, medical questionnaire sheet is distributed at the
time of visiting babies.
