Information of vaccination against COVID-19
Vaccination is conducted sequentially according to schedule. This article shows
the issuance method for Vaccination Certificate of COVID-19 (Vaccination
Passport) and frequently asked questions about vaccination.

Issuance of Vaccination Certificate of COVID-19 (Vaccination Passport)
Applications have been accepted for Vaccination Certificate of COVID-19
(Vaccination Passport) which is available for overseas use. Infectious
preventive measures will be eased as shortened quarantine segregation period
at entry to some countries, and exemption of submitting PCR Test Negatrive
Certificate before departure etc. by showing the Vaccination Passport.
Eligibility: Person who took vaccination with vaccination tickets issued by
Toyokawa city, has a passport with validity period and intention to go abroad
Expense: Free of charge
Application: Bring directly or send by post an application form with copies of
necessary documents (Passport, vaccination ticket, Certificate of Vaccination
for COVID-19 or Record of Vaccination for COVID-19) to Health Center (〒442-
0879 Toyokawa Shi, Hagiyama Cho 3 Chome 77-7) For sending by post,
enclose a return envelope with delivery address and 84 yen stamp on it.
Application form is available at Health Center (It can be downloaded from city
HP.).
Others: It might take some time for issuance of the certificate. Please apply
with enough time for your schedule. One certificate is issued per application.

As National Health Insurance Premium is source of revenue for the insurance
benefit, therefore, it is very important to make payment by the due date without
fail. For this payment, account transfer payment is convenient.
■If it is kept unpaid
Except special case such as disaster, if you continue non-payment of the premium,
the following measures are conducted.
1) Your health insurance benefit from high-cost medical care and other benefits
2) will be allocated to your unpaid insurance payment.
3) Depending on period of the unpaid premium, the validity term of your health
4) insurance certificate will be shortened, so called “short term health insurance
5) certificate” will be issued instead or “insured qualification certificate” is issued
6) which you have to pay all the medical bills each time temporarily.
3) Seizure of property may be conducted.
■ Getting or losing the qualification of “National Health Insurance”
1) When you leave your job or newly move into Toyokawa from other places,
please apply for the qualification of “National Health Insurance”, and the
insurance premium becomes effective. If application is delayed, you may have
to pay the insurance premium which is calculated back to the day of qualification
occurrence, so please complete the registration WITHIN 14 DAYS.
2) The insurance premium will be imposed on householder. A householder is the
tax debtor for national health insurance. Even though the householder has the
Social Health Insurance, in case of the family members belonging to National
Health Insurance, the notification is addressed to the householder.
