About application for High-Cost Long-Term Care Unitary Medical

Application acceptance starts for High-Cost Long-Term Care Unitary Medical
Care Benefit. This benefit is to reimburse the excess annual cost of Medical
Insurance and Long-Term Care Insurance. This time, the payment is for the
period of August, 2022 to July, 2023. The notification is sent to the eligible
person (For person insured by National Health Insurance (Kokumin Kenko
Hoken), in the end of February, and for person insured by the Late-stage
Medical Care System for the Elderly (Koki Koreisha Iryo Seido), in the end
of March.). There may be cases that notification cannot be sent to the
person who changed status of medical insurance, composition of household
members etc. during the period. For person who joins other medical
insurance than National Health Insurance and the Late-stage Medical Care
System for the Elderly, please contact respective medical insurance
organization.

Eligibility: Child who was born between April 2, 2017 and April 1, 2018
without vaccination
Fee: Free of charge
Application: Call the commissioned medical institutions written on City HP.
In case of taking vaccination at medical institutions in other municipalities,
procedures at Toyokawa Health Center (Hoken Center) are necessary in
advance. Please come directly to Toyokawa Health Center with maternity
passbook.
Others: Charge will occur after March 31, 2024. If you do not have medical
questionnaire sheet for vaccination, contact Health Center (Tel: 0533-95-
4801).

Eligibility: Person aged 6 months or older who has completed 1st/2nd
vaccination
Eligible vaccination tickets: Vaccination tickets (cream color)
※ First vaccination is also implemented for person who has never taken. In
case of wishing to take vaccination, contact Toyokawa City Novel Corona
Vaccine Call Center.
※From the fiscal year 2024 onward, periodical vaccination is planned
principally for the elderly. Therefore, current free vaccination will not be
available. If you wish to take the vaccination, please consider taking it in
the above-mentioned period.
